Introduction: Increased production of free radicals following physical stress leads to oxidative stress. Post-coronary angiography care, including immobilization and the use of sandbags to restrict movement, often causes fatigue and back pain in many patients. Given the immobilization and associated physical discomfort, this study aimed to investigate the effect of early mobilization on the levels of malondialdehyde (MDA), as a marker of oxidative stress, in patients undergoing coronary angiography. Methods: In this randomized clinical trial, 50 patients undergoing coronary angiography were randomly allocated into an intervention group (n=25) and a control group (n=25). In the intervention group, the duration of sandbag application was reduced from 6 hours to 4 hours. The control group received routine care (sandbag application for 6 hours). Malondialdehyde levels were measured using standard kits from blood samples taken before angiography and 6 hours post-procedure. Data were analyzed using SPSS software (version 22) and statistical tests including independent t-test, paired t-test, ANOVA, and Chi-square. Results: The results showed no significant difference between the intervention and control groups regarding baseline and demographic variables (p > 0.05). Before the intervention, the mean malondialdehyde levels in the intervention group (33.0 ± 9.68) and the control group (31.69 ± 12.01) did not differ significantly (p = 0.63). After the intervention, there was also no significant difference in the mean malondialdehyde levels between the intervention group (31.74 ± 10.34) and the control group (30.49 ± 10.86) (p = 0.80). Conclusion: Based on the findings of this study, reducing the duration of bed rest from 6 hours to 4 hours does not significantly affect malondialdehyde levels as a marker of oxidative stress in patients undergoing coronary angiography. Therefore, further studies are recommended in this field.
